PGY1 - Pharmacy Residency Program in the Ambulatory Care Setting
Mission Hospital and the Mountain Area Health Education Center (MAHEC) work jointly to provide core sites for the Residents.
MAHEC
MAHEC is part of the North Carolina AHEC system that was established in 1972 to improve the supply, retention and quality of health care across the state. MAHEC's mission is to provide education, information, training, and services to meet health needs in Western North Carolina through collaborative relationships. MAHEC is home to the Family Practice Residency Program and the OB/GYN Residency Program. Faculty pharmacists within the Department of Pharmacotherapy coordinate continuing education for regional pharmacists, teach many learners (pharmacy residents, family medicine residents, geriatric fellows, dental residents and PY4 Doctor of Pharmacy students), and provide medication management for both inpatient and outpatients.
Direct patient care is provided for patients with chronic diseases and polypharmacy. Primary care residents will provide longitudinal care for a panel of patients under the supervision of faculty pharmacists and will develop a mastery of skills and knowledge needed to practice in primary care. Residents will work side-by-side with medical residents, faculty physicians, and behavioral medicine specialists. Pharmacotherapy and behavioral medicine are integrated into family medicine using the integrated care model.
Mission Hospital Health Education Center
The Mission Hospital Health Education Center offers comprehensive education and disease management services. These services include an ADA-recognized diabetes education program, hypertension classes, health coaching, weight loss classes, and nicotine dependence programs. Care management services are based on the Asheville Project® model and include diabetes, hypertension, hyperlipidemia, depression, asthma, and warfarin management.
The Center is staffed by nurses, a respiratory therapist, social workers, pharmacists and dietitians, many of whom are Certified Diabetes Educators (CDE). Pharmacy residents have the opportunity to manage a group of patients throughout the year and to teach group classes.
Mission Medication Assistance Program (MAP)
The Mission MAP is an excellent opportunity to experience first hand the challenges of obtaining medications for low income populations. This program, which has been recognized by the American Public Health Association as an Innovative Practice, provides intensive medication management, disease management education and overall health counseling to high risk community members.
Mission Outpatient Clinical Pharmacy Services
Mission Hospital Outpatient Clinical Pharmacy Services began as an expansion project of Mission Hospital Outpatient Anticoagulation Clinic in 2005 and encompasses a variety of services. The mission of the clinic is to provide excellent pharmaceutical care by enhancing patient understanding of medication and chronic diseases; and to promote overall health and well being through medication and disease state management in collaboration with their health care providers. Mission Outpatient Clinical Pharmacy Services provides anticoagulation and medication therapy management services for various high-risk populations. The pharmacists, pharmacy residents, and pharmacy technicians in the clinic also facilitate continuity of care by assisting with medication reconciliation during transitions in care.
