Help for Chronic Medical Conditions

Take charge of your medical condition

Mission's My Healthy Life program offers people with health challenges and chronic conditions a way to live the healthiest life possible - and meet others in the process. Our programs are available through many Western North Carolina employers, or you can join on your own.  You'll be in good company - more than half of adult Americans now have health issues such as overweight or high blood pressure, or a chronic illness such as diabetes.

Our programs can educate and assist you with managing conditions and diseases like:

We can provide individual dietary counseling tailored to your specific illness or condition and nutritional needs.

Weight management programs through our Health Education Center provide the information and support you need to succeed. Private and group options are available.

Seniors (over 65) taking multiple medications can get a free "medicine check-up" every year, during which a pharmacist will look at all your medicines and help you with side effects or drug interactions. CheckMedsNC is a state program, free for people over the age of 65 on the Medicare (Part D) drug plan.

Disease management and wellness programs may be available through your employer. If so, you'll be paired with a care manager "coach" for one-on-one help in managing your chronic condition or disease. These programs are based on The Asheville Project®, a nationally recognized disease management model. It has given participants better quality of life, less absenteeism - and lower medical bills.

Check Your Insurance
We recommend that you check with your insurance provider before beginning any of the My Healthy Life programs. Some programs may require a doctor's referral for you to participate or for your insurance to provide coverage.
