For Referring Physicians:
By referring your patients to the Asheville Specialty Hospital you are ensuring that your patients receive ongoing care.
Patients who have suffered a catastrophic illness and are anticipating a lengthy hospitalization are appropriate referrals.
Bed availability for new patients coming to the main hospital can frequently be a problem, therefore patients identified as needing a prolonged hospital stay should be referred to ASH and evaluated by a Referral Liaison.
An Asheville Hospitalist physician frequently assumes the care of referred patients. As the prior attending physician, you become a consulting physician to the Hospitalist, allowing more time to see your other patients.
Admittance to ASH is made by the physician and facilitated by the Case Manager and/or Discharge Planner. The process is as follows:
-
Order entered by physician.
-
Call ASH Referral Line at 213-3400 and leave a message.
-
Referral Team nurse will return your call within an hour, evaluate the patient that same day, and determine if they meet the admission criteria. This visit is documented in the physician's progress records.
-
If accepted, Referral Team nurse works with the Multidisciplinary Team, including the Case Manager/Discharge Planner to arrange admission.
-
If declined, the reasons are communicated to the Case Manager along with documentation in the physician's progress records.
